FREEDOM OF INFORMATION

The Governance Commission strictly adheres to the policy of the State on the Freedom of Information (FOI) as enshrined in Executive Order (E.O.) No. 2, series of 2016. To efficiently carry out the mandate of the said law, those who wish to request documents/information from the GCG may download the FOI Request Form below.

For FOI Requests, please submit the following to the GCG Office or via e-mail at feedback@gcg.gov.ph:

1. Completed FOI Request Form;

2. Authorization for Representative; and

3. One (1) Valid Government – recognized ID.

Please note that the turnaround time for FOI Requests, based on Section 9(d) of E.O. No. 2, is fifteen (15) working days from GCG's receipt of the complete requirements stated above.
